You're shown a prompt asking if you like the app. If you say yes, you're brought directly to the app store to leave your 5 star review. If you say no, no such redirection takes place; you get a sincere looking form to describe your misgivings, but mainly to keep you from writing a bad review. This way, the devs can cherrypick exclusively good reviews. I wonder if anyone even looks at the naysayers' feedback forms?
